There is a NEW Online USA-Swimming Incident Report.  A link has been posted on the Hawaiian Swimming | Documents page.  The form can be filled out on a smart phone, iPad or computer with network connection.  A fillable and saveable .pdf file has also been posted to print, or to keep on non-connected devices.
